How can that be done "Keep the TH and OH sounding at the same time. You should be making the TH sound all throughout the exhale" ?

Make the TH sound while rounding your lips as if you're trying to say "OH", this way you vibrate both "TH" and "OH" at the same time.

Its definitely possible. It sounds odd when you finally get it right but you'll know. I had more trouble getting the hand position right.
This is an exercise that helped me. Vibrate "AUM" slowly. And focus on the feeling in your throat at the 'm' and try to replicate that feeling when vibrating Thoth.
The key thing as well is experimentation
