This kind of thing can literally happen it is no different from changing reality through energy manipulation and magick.
Similar things happen to a lot of superstitious people. It may not even necessarily be a physical manifestation, objectively visible, but to him it may be visible.
Here is the reason: Through your superstition and expectation you are raising a certain type of energy and constantly directing it at the expected outcome = the outcome occurs.
I experienced a similar thing with the lines on my palms changing.

I don't know if you're lying or telling the truth, but I have had a symbol visibly appear on my hand before as well. I had one that looked like a "G" and my friend related it to a letter in an apparently magickal language that I can't remember the name of. Magi or something like that. Anyways it was very weird. It just showed up on my hand one day I was at a friend's house doing meditations and such, colored red very clearly and visible to my friends as well. At the time I was doing so much chi work I literally electrocuted my hands by overloading them with bioelectricity, giving them firstly a patchy look on the underside and then turning the backs of my hands hard like reptile skin as if I had eczema. Went away when I took a break for a week. Anyways If you got a pentagram on your hand and you're telling the truth, then it must be a sign that you're on the right path or something like that. Whatever feeling it gave you essentially is the message that you should be receiving from it in my opinion.
