I think I can help you with it. I was like you and still am sometimes and its hard to overcome it fully but it gets so easy after awhile. so first thing you need to do is that you should focus on present moment and not think about for example what you are going to do after two hours . the other problem is being worried about things and you should try to imagine things so simple to handle and believe it deeply in your heart that you are going to do that thing at this time perfectly. its a habit as all habits when you replace it with doing things you always do. the other thing is don't think about it just do it. its maybe hard but you get used to it (just do . don't think)

Your problem lies with focusing I think. I'm the same sign. It's not always "just laziness". A person can become very dispersed. By that I mean, they can start on one thing in their mind, then start on another thing in their mind, then another and so on until they really don't know what they are doing and then they feel weak, overwhelmed, hopeless and they blow off what they really wanted to get accomplished, only to feel really bad about it. And they keep repeating that. I did that for years and could never figure it out either.
You have to get it in your head that you are going to do one thing and that one thing is the best thing in the world that you can do for that 13 minute period of time. I choose 13 minutes to work on a task whenever their are too many things to get done.
To backtrack a little bit. Go to Excel or your word processor or simply a piece of paper and write down the date on the top of the page and write down everything you have to do on that date or would like to do. If you use Excel its very easy to make dates and write down whatever you've got to do on a certain day.
Organization is really helpful. Organize like tasks, so they're grouped together. For me I use a four column Excel spreadsheet with at least a couple years worth of dates and days on it. So, its, Date, Day, Two or Three word description, and finally the full description of whatever it is you plan to do. Leave one or two rows below the todays date and day and just fill the two columns down for a couple years. Freeze a title at the top, for the column headings and you're all set. Update this daily and never forget. Print it out in the morning every morning and don't ever forget it. (Print selected text option).
For example, today I had to read some of 4 different books, clear my chakras, open my chakra, and build my aura of protection in addition to figuring out what to do with a troublesome Poo Angel and some zombie xians. I had to remember to get groceries immediately after work. You get the picture. If I worked on what I think I'm supposed to, I would never get done with my Satanic Witchcraft stuff. I don't think its procrastination.
Try to get your list of things done. 13 minutes is a good number. Just do the best you can at choosing the item to do that would make you the most happiest. Don't pick the most productive, because it usually it sucks. You'll get to it, if you simply don't try to spend to much time on one thing or try to do everything all at once.
Having a fixed time to do everything sucks. Be spontaneous and keep your Satanic Witchcraft stuff the priority and you'll always get everything done.
