Apparently Sorath sign is the 13 zodiac sign which is ophiucus. That means the physical 13 sign system is actual one that should be used and changes all my personal planets. What system shoul be used?

No. You have to use the one in the ToZ, with 12 signs. The inclusion of Ophiuchus doesn't change anything.

I used a website to generate the 13 sign system on my birthday and my intuition is telling me hard the planets do not fit at all. Im agreeing with you that the original system used in the TOZ is very most likely the correct one.

We actually have 88 constellations around us. The 12 Zodiac constellations, and Ophiuchus touches a bit within, but the energy of the 12 we use are the ones that work for natal charts.
