...positive clauses be embedded into an affirmation?

Not that I'm doing any magick at the moment as I simply don't want anything. But the question is when should they be added?

Is a affirmation without a positive clause ALWAYS gonna end up bad. What if getting out of your comfort zone is bad but not horrible or anything, is that what we want?

My go to is (safe, healthy, happy, and positive way for me). But I noticed in recent times some people mentioning affirmations without positive clauses or if they add positive clauses it's only like 1 thing like say positive or beneficial or whatever. I've even notice some of Maxine's affirmation not stating a positive clause. Not that she doesn't have positive clauses in her affirmations like the money magic ritual. But I notice short and the point affirmations that bear no positive clauses.

Exactly how should we "program" the raised energy knowing above what I said?

If you are not confident in your intentions to properly direct the energy of the affirmation in affirming the goal in a safe, healthy and positive way for yourself, then you should add positive clauses to the affirmation to assert it.
