I did the dedication ritual 1.5 years ago and remembered that I didn't write my birth name and only the name I was suppose to be given at birth. I'm a boy but I have a girl's name because my mom tried too hard at the english accent when the lady asked her for my name :'(. 
Anyway to this day I'm 17 years old and I still have a girls name but I still tell everyone to call me by the male version, should I rededicate my soul to Satan and use my birth name(girl name)?.

I believe that if that is the name you are known by, then it is valid. If I am wrong, surely someone will correct me.
