The 12th house is self-undoing, hidden enemies, and abstractly-speaking that enemy may also be yourself. You may feel higher self-esteem once you find an unconditionally loving partner who is attracted to you on the inside and outside. Another way to look at this, is you are destined to outgrow this low self-esteem. The hidden enemies who spoke of you behind your back, or whoever else, were the ones who made you insecure on a subconscious level.
The other form of ugliness is just harsh negative emotions and bad behaviors of your own or other people. Neptune in its home house also means powerful spiritual evolution is possible in this lifetime. Rise above this modern world's unfair expectations of beauty.
My partner can be determined as "ugly" by anybody, but I see him simply beautiful as he is, because his face and scars represent his confidence, strength, and determined attitude. Perhaps someone out there will have a meaningful interpretation of your own appearance as well.

I have similar descriptions in my chart and it has nothing to do with your appearance. It's a warning for getting involved with the ugly sides of the world. You should avoid things than turn your stomach, make you feel unsafe/uncomfortable, and general high amounts of violence, depression, torture, etc. Don't hang with people who are huge downers/suicidal, as this can rub off on you. It's important for you to create a stable, positive environment for yourself. Negativity doesn't bounce off of people like us easily. Don't let yourself stew in it.

The 12th can also deal with self-imprisonment and the endless attainment of perfection (think something like a doll, which Neptune rules) which on the lower level of 12th/Pisces can be delusional. Persistent beliefs that you're too ugly to participate in society or reinforcement of this from enemies are part of this deception.
This also goes for the 6th House sometimes.
