Saturn in 4°house has various effects, poor family, Problems whit parents, family Severity, death of a parent, bad relationships, negative influences/conditions , psychological problems, in some cases violence, abuses and damage to the body, in rare cases death due to family, etcc...
But it is neccesary to look whole birth chart . And remember that Saturn is not only "negative".
